ACE Inhibitors Increase Trasylol Risk

A new report from British researchers is showing that the risks of kidney failure when using Trasylol increased when also used with ACE inhibitors during off-pump open heart surgery. Off-pump surgery meaning the heart is beating and no bypass machine is used. Patients that received Trasylol during surgery on a bypass machine had no increase risk of injury. Aprontinin is the generic name for Trasylol.

Among patients who had on-pump surgery, there was no significant association between aprotinin and postoperative renal dysfunction, irrespective of ACE inhibitor use. However, aprotinin was associated with a more than twofold increased risk of renal dysfunction among the 848 patients who had off-pump cardiac surgery and received both aprotinin and ACE inhibitors.

"We recommend that it might be beneficial for patients to discontinue any use of an ACE inhibitor before elective off-pump surgery, particularly patients with a history of renal impairment," wrote Dr. Ronelle Moulton and Kai Zacharowski, from the department of anesthesia, Bristol Royal Infirmary, United Bristol Healthcare Trust.

Results from the BART study in Canada showed a signficantly high risk of kidney failure, heart attack and strokes following the use of Trasylol. Trasylol was pulled from the market after the study was halted. Further analysis of the data from BART study is being done.
